Forma provides general fitness and nutrition information. It is not medical, dietary or physiotherapy advice and is not a substitute for a qualified professional. Talk to a doctor before starting a new exercise or nutrition program, especially if you have a health condition, injury, are pregnant, or take medication. Stop exercising and seek help if you feel pain, dizziness or shortness of breath.

AI-generated content (coaching, plans, recipes, logged-set interpretation, nutrition values) can be wrong. Check allergens and ingredients yourself — never rely on Forma alone to avoid an allergen.

Eligibility

You must be 18 or older and provide accurate information.

Your account

Keep your password private. You're responsible for activity on your account. You can delete your account at any time in Settings.

Forma Pro

Forma Pro is a subscription billed monthly or yearly at the price shown when you subscribe, with a 14-day free trial for new subscribers. It renews automatically until cancelled. Cancel at least 24 hours before the trial or period ends to avoid being charged. Purchases made through the App Store or Google Play are managed and refunded under their policies.

Acceptable use

Don't misuse the service, try to access other people's data, reverse-engineer it, or use the AI features to generate harmful content.

Your content

You own what you log and upload. You give us permission to store and process it only to run Forma for you, as described in our Privacy Policy.

Third-party content

Exercise videos are provided by YouTube and food data by Open Food Facts. We don't control and aren't responsible for that content.

Liability

Forma is provided "as is". To the extent the law allows, we aren't liable for injuries, health outcomes or indirect losses arising from use of the app, and our total liability is limited to what you paid us in the 12 months before the claim. Nothing here limits rights you have under consumer protection law.

Changes and governing law

We may update these terms and will note the date above. These terms are governed by the laws of British Columbia and Canada.
